Proctored Test Reservation Form

If you are a student who has an Instructional Accommodation Plan (IAP) which provides an accommodation for tests and exams, you may schedule a proctored time and space by completing this form. Please schedule a minimum of three business days before the test or exam. In times of high volume of tests and exams, Disabilities Support Services (DSS) may have to schedule alternate days and times from your request. We recommend that you schedule as early as possible.
